Position Summary:

In this critical and fulfilling role as aWoundOstomyNurse, you will provide patientwoundcarein an acute setting, reporting to physicians any changes in patient’s condition, with an emphasis on assessment, illness prevention and healthcaremanagement. TheWoundOstomyNursemust be able to respond quickly to changes in condition and assist physicians. This self starter will also act as liaison between patients, physicians and other healthcareproviders.

Duties and Responsibilities:

· Knowledge of medications and correct administration, reducing the potential for medication errors
· Initiating the clinical pathway on admissions; collects patient history and develops nursing assessment; reviews charts, lab reports and other diagnostic data; contacts clinician to get orders
· Reviews assesses, measures and evaluateswounds andwoundprogress; interprets diagnostic data; assigns grade and stages ofwounds; photographs and determines appropriate dressings, applies and changes dressing as required
· Interprets data to determine when a change incareis appropriate and makes decisions regarding treatment changes defined within scope of responsibility.
· Participates in rounds/examinations conducted by physician/clinician; develop and confirmcareplan; documentswoundcaretreatment and identifies next step for the patient in accordance wit the established clinical pathway and overall plane ofcare
· Assist withwounddebridement and otherwoundcaremodalities
· Responsible and accountable for the assessment, planning, implementation, and evaluation ofwoundand Ostomy patients
· Provides direct patientcareand education to selected patients in accordance with the State’sNursePractice Act, physicians orders, recognized nursing techniques, policies, procedures, established standards and administrative policies.
· Provides staff development in areas of expertise and is responsible for product recommendations in areas of expertise.
· Able to perform head-to-toe assessment on all patients, formulate an individualized plan ofcareand evaluate plan daily for effectiveness
· Able to assess andcarefor patients in ICU
· Able to accurately assess pain and utilize appropriate pain management techniques
· Implement and maintain established policies, procedures, objectives, quality assurance, safety, and environmental and infection control. Interpret these to the physician, resident, family members, and public, as appropriate.
· Communicate with chargenurse, coworkers and physicians, consulting other departments to provide an interdisciplinary approach to patients' needs
· Maintain documentation that meets current standards and policies.
· Assist the Quality Assessment & Assurance Committee in developing and implementing appropriate plans of action to correct identified deficiencies.
· Meet with and solicit advice from the Medical Director, attending physicians, Director of Clinical Services, and nursing staff concerning thecareof patients.
· Maintain an effective, friendly working relationship with health professionals, physicians, consultants, and family members involved.


Requirements:
· Must have 2-5 years ofwoundcareexperience
· Must have a CWOCN
· Must complete ACLS certification within 6 months of hire.
· Candidate must be flexible, organized and function well in stressful situations.
· Current state RN License required along with current BCLS certification.
· Must successfully complete background check and drug screen.
